Privacy Notice

I understand how important it is to protect your privacy and I want to ensure that you can have trust and confidence in the way I manage your personal information. I always ensure that personal information given to me by my clients is processed lawfully, fairly and in a secure and accurate manner. I only collect personal information for the reasons outlined within this Privacy Notice and information will not be processed in any way other than the methods described below.

When I collect information

I gather and process your personal information when you:

apply for a consultation
share information with me during a consultation
contact me through my website , social media, email, by telephone or letter
sign up to my mailing list

Depending on the nature of how you are accessing my services, the information I collect about you may include:

personal information such as your full name, date of birth, address, religion and nationality. The provision of your nationality, religion or any other form of Sensitive Personal Data is at your own discretion and may be withheld from my records if you prefer
contact information including your email address and telephone number
medical information and history, such as but not exclusively, your current GP, any medication you are taking, any other treatments you are (or have been) under going, any past accidents, injuries or surgeries, any intolerances or allergies you suffer from. I use these to formulate a safe, appropriate and effective treatment plan for you
dietary information. I use this to formulate a safe and appropriate treatment plan for you
preferences you've chosen regarding communications. I collect these so I know how you would like to be contacted and what I can talk to you about

Why I collect your personal information and how I use it

I use the information I collect about you to:

formulate a prescription that is appropriate to your needs
retain files and notes for accurate record keeping
send you consultation notifications or reminders
contact you with information relevant to your appointment
request an update on how your treatment is progressing
send you relevant medical information in relation to you and your health
send you invoices and/or chase money owed
improve my services, so that I can continue to develop services that meet the needs of my clients
send you information (if you agree) about any product updates, promotions, or surveys.

Protecting Your Information

I protect all of your information against unauthorized access or release. Your personal data will be treated as strictly confidential, and will not be shared with or transferred to any third parties. In some instances we may agree that a referal to an additional or another practitioner relevant to your health care needs is required; where this is the case you will be consulted prior to the referal being made and I will obtain your consent before sharing any information. I will then share only the information which is relevant and appropriate in the case of each referal.

Please note that as a Clinical Herbalist I may in very rare circumstances be required to share information with another health service or the police as part of my 'duty of care'.

I take the security of your personal information seriously. I have technical controls and procedures in place to ensure that all personal information is processed and stored in a secure and confidential manner. All paper records are kept in a secure manner and electronic records are encrypted.

How long I'll keep your information

I keep your personal data for no longer than reasonably necessary. In order to comply with accounting practices and legal requirements it is compulsory that I keep your data for a period of at least 7 years from our last contact.
